Training Problems

Q: My puppy is 12 weeks old and she was doing so well with pad training that I also introduced her to our backyard. She started eliminating almost immediately. After about a week of using both the pad and outdoors, she has had several mishaps in the house including on the carpet and wood floors. She seems to be regressing. What should I do?
